New Patient Forms

Patient Forms – As a new patient, we need information about you and your health history.  Please print the forms, fill them out and bring them with you to your first appointment. 

Notice of Privacy Practices – This notice describes how chiropractic and medical information about you may be used and disclosed and how you can get access to this information.  Please review it carefully

HealthCare Authorization Form – This document states that you have been provided with a copy of our Notice of Privacy Practices (above) and also addresses specific ways in which we may use or disclose your Protected Health Information.  Please review, print and sign this document and bring it with you to your first appointment.

Consent to Treat a Minor – If the new patient is under the age of 18, we need parental or guardian permission to examine and provide care for your child.  Please print this form, fill it out for your child and bring it with you to their first appointment.

Accident Papers – As a new or existing patient consulting our office as a result of a recent work related injury or a motor vehicle accident, we need additional information.    Please print the forms, fill them out and bring them with you to your appointment.
